Estudos de Saúde Mental publicados nos últimos 25 anos na Revista Ciência & Saúde Coletiva / Mental Health Studies published in the last 25 years in the Journal Ciência & Saúde Coletiva

Resumo Realizou-se revisão sistemática de 25 anos de produção da Revista Ciência & Saúde Coletiva sobre a temática de Saúde Mental buscando responder se essa produção tinha se modificado ao longo do tempo. Perguntou-se se essas mudanças guardariam alguma relação com a implementação de um novo marco regulatório e legal para essa área assim como com a expansão de serviços públicos e com os estímulos à investigação do tema por Agencias Públicas de financiamento. Analisaram-se 278 artigos originais, apresentados a partir das categorias: transformações socioculturais relacionadas à loucura e sua abordagem; mudanças legislativas; implantação, capilaridade e funcionamento da rede de serviços substitutivos; clínica/cuidado desenvolvido nos serviços substitutivos; o papel e as possibilidades da atenção básica; problemas relacionados ao uso de drogas; saúde mental de crianças e adolescentes; estudos epidemiológicos/categorias psiquiátricas; e outros - nas quais foram incluídas as questões étnico-raciais, de violência, acerca da população idosa, suicídio, bullying e migração. Pode ser observada relação cronológica entre o aumento de publicações e os editais de pesquisa e a expansão de serviços, não parecendo ser da mesma forma em relação ao tema dos cuidados a pessoas com problemas com álcool e outras drogas.

Abstract A systematic review of the 25-year Mental Health production of Journal Ciência & Saúde Coletiva was performed to identify whether this production had changed over time. We investigated whether these changes would have any relationship with the implementation of a new regulatory and legal framework for this field and the expansion of public services and the promotion of public funding agencies' investigation on the subject. A total of 278 original papers were analyzed from the following categories: sociocultural transformations related to madness and its approach; legislative changes; implantation, coverage range and operation of the network of substitute services; clinic/care developed in substitute services; the role and possibilities of primary care; drug use-related problems; mental health of children and adolescents; epidemiological studies/psychiatric categories; and others - which included ethnic-racial issues, violence, about the elderly population, suicide, bullying, and migration. A chronological relationship can be observed between the increase in publications and research notices and the expansion of services, not seemingly in the same way concerning care for people with alcohol and other drug problems.

A pesquisa avaliativa do processo de formulação e implantação da Política Nacional de Atenção Integral à Saúde da Criança (PNAISC): trajetória epistemológica e metodológica / The research of the formulation and implementation process of the National Policy for Comprehensive Child Health Care (PNAISC): epistemological and methodological trajectory

O processo de formulação e implantação da Política Nacional de Atenção Integral à Saúde da Criança (PNAISC), iniciado no final de 2011, com a coordenação da Área Técnica de Saúde da Criança e Aleitamento Materno (ATSCAM/CGSCAM) do Ministério da Saúde (MS) e da Estratégia Brasileirinhas e Brasileirinhos Saudáveis (EBBS), foi acompanhado por pesquisa avaliativa desenvolvida a partir da triangulação de métodos quantitativos e qualitativos sob o referencial epistemológico da hermenêutica-dialética. Este artigo discute as diferentes abordagens da avaliação e os diferentes posicionamentos em relação à combinação de métodos e epistemes. Para isto, recupera a discussão realizada por Heidegger sobre o conceito grego originário do matemático (µαθήµατα) e sua apropriação por Descartes na formulação de sua filosofia (mathesis universalis). Aponta e justifica a escolha epistemológica e metodológica dos autores e apresenta o desenho da pesquisa, que objetivou vislumbrar diferentes aspectos do processo de formulação e implantação da PNAISC: as tradições que informam sua formulação, o desenrolar do processo de formulação e implantação e o contexto nos diversos territórios

The formulation and implementation process of the National Policy For Comprehensive Child Health Care (PNAISC), which started at the end of 2011, with the coordination of the Technical Area of Child Health and Breastfeeding (ATSCAM/CGSCAM) of the Ministry of Health (MS) and of the Estratégia Brasileirinhos e Brasileirinhas Saudáveis (EBBS), was accompanied by evaluative research developed from the triangulation of quantitative and qualitative methods under the epistemological framework of dialectic hermeneutics. This article discusses the different evaluation approaches and the different positions regarding the combination of methods and epistemics. For such, it recovers the discussion carried out by Martin Heidegger on the original Greek concept of mathematics (µαθήµατα) and its appropriation by Descartes in the formulation of his philosophy (mathesis universalis). It points out and justifies the authors' epistemological and methodological choices and presents the design of the research, which aimed to glimpse different aspects of the formulation and implementation process of the PNAISC: the traditions that inform its formulation, the course of the formulation and implementation process, and the context in the several territories

Estudo das tradições que informam a Política Nacional de Atenção Integral à Saúde da Criança (PNAISC) / Study of the traditions that inform the National Policy for Comprehensive Child Health Care (PNAISC)

No final de 2011, a Área Técnica de Saúde da Criança e Aleitamento Materno do Ministério da Saúde iniciou discussões para formulação e implantação da Política Nacional de Atenção Integral à Saúde da Criança (PNAISC) e definiu pelo acompanhamento avaliativo desse processo desde seu início. Apoiando-se no referencial da hermenêutica-dialética, a pesquisa objetivou vislumbrar diferentes aspectos do processo de formulação e implantação da PNAISC: as tradições que informam sua formulação, o desenrolar do processo de formulação e implantação e o contexto nos diversos territórios. A partir do contato com os principais atores desse processo, foi possível, para a equipe de pesquisa, vislumbrar um elenco de tradições que o influenciavam. Para o estudo hermenêutico dessas tradições, foi criado um Filtro de Estudo, aplicado às seis principais tradições, que permitiu elaborar Sínteses Compreensivas e facilitar sua análise comparativa. Foi construída uma Matriz das Tradições, destacando os seus princípios, diretrizes, arranjos e dispositivos. O estudo estabeleceu a trajetória das tradições que informam o processo de elaboração da PNAISC e como cada tradição influenciou e/ou foi influenciada pelas demais, sendo elaborados Mapa Hermenêutico das Tradições e uma narrativa desse percurso. Em seguida, foram colocados em destaque os principais conceitos estudados, traçando a sua trajetória. Assim, foram desenhados oito Mapas Hermenêuticos e suas narrativas: Mapa Hermenêutico das Tradições da PNAISC e os Mapas com as trajetórias dos conceitos: Democracia Institucional, Clínica Ampliada, Apoio Institucional, Articulação Interfederativa, Redes de Atenção, Intersetorialidade e Saberes Tecnológicos envolvidos

At the end of 2011, the Technical Area of Child Health and Breastfeeding of the Ministry of Health initiated discussions for the formulation and implementation of the National Policy for Comprehensive Child Health Care (PNAISC) and defined the evaluative accompanying of that process since its beginning. Based on the framework of dialectic hermeneutics, the research aimed to glimpse different aspects of the process of formulation and implementation of the PNAISC: the traditions that inform its formulation, the course of the process of formulation and implementation, and the context in the different territories. From the contact with the main agents of that process, it was possible, for the research team, to glimpse a set of traditions that influenced it. For the hermeneutic study of those traditions, a Study Filter was created, being applied to the six main traditions which allowed to elaborate Comprehensive Syntheses and facilitate its comparative analysis. A Matrix of Traditions was built, highlighting its principles, directresses, arrangements, and devices. The study established the trajectory of the traditions that inform the process of elaboration of the PNAISC and how each tradition influenced and/or was influenced by the others, being elaborated a Hermeneutic Map of Traditions and a narrative of that course. Next, the main concepts studied were highlighted, tracing their trajectories. Thus, eight Hermeneutic Maps and their narratives were designed: Hermeneutic Map of the Traditions of the PNAISC and the Maps with the concepts' trajectories: Institutional Democracy, Extended Clinic, Institutional Support, Inter-federative Articulation, Care Networks, Inter-sectoriality, and Technological Knowledge involved

Tecendo a história da construção da Política Nacional de Atenção Integral à Saúde da Criança (PNAISC) na visão dos sujeitos envolvidos: o desenho qualitativo da pesquisa com utilização da técnica de grupo focal / Weaving the history of the construction of the National Policy for Comprehensive Child Health Care (PNAISC) in the perspective of the subjects involved: the qualitative design of the study with use of the focal group technique

Esta pesquisa nasceu de uma demanda da Área Técnica de Saúde da Criança e Aleitamento Materno do Ministério da Saúde por instaurar um acompanhamento das ações desencadeadas no processo de formulação e implantação da Política Nacional de Atenção Integral à Saúde da Criança (PNAISC), que se constituísse como dispositivo de análise e aprendizagem, subsidiando possíveis ajustes e aportes na trajetória em curso. A pesquisa pretendeu avaliar diferentes dimensões do processo de formulação e implantação da política, compreendendo em diferentes estudos: as tradições que informam sua formulação, o desenrolar do processo em si de formulação e implantação e o contexto nos diversos territórios para sua implantação. Neste artigo, apresentamos o estudo do processo de formulação e implantação da PNAISC, realizado por meio de grupos focais e construção de narrativas. Este estudo compreendeu 19 grupos focais, com a participação dos diversos atores integrantes do processo, sendo elaboradas narrativas a partir das transcrições desse material. Essas narrativas foram processadas em grades temáticas, destacando o que foi discutido em quatro eixos temáticos: Apoio, Articulação Interfederativa, Rede Cegonha (RC) e Arranjos e Atores da PNAISC. Os discursos dos principais atores da pesquisa situados nesses quatro eixos temáticos constituíram a Matriz das Diversas Vozes, que foi apresentada e validada em uma Oficina da Pesquisa Avaliativa, com os diversos atores envolvidos no processo de formulação e implantação da PNAISC. Nessa Oficina, foi formulado um elenco de potencialidades, fragilidades e proposições para cada um dos eixos temáticos. This study originated from a demand from the Technical Area of Child Health and Breastfeeding of the Ministry of Health for instituting an accompanying of the actions triggered during the process of formulation and implementation of the National Policy for Comprehensive Child Health Care (PNAISC), which would be constituted as a device for analysis and learning, subsidizing possible adjustments and contributions on the trajectory in progress. The research intended to evaluate different dimensions of the process of formulation and implementation of the policy, comprehending in different studies: the traditions that inform its formulation, the course of the process of formulation and implementation itself, and the context in the different territories for its implementation. In this article, we present the study of the process of formulation and implementation of the PNAISC, carried out through focal groups and narrative construction. This study comprehended 19 focal groups, with the participation of the many agents of the process, elaborating narratives from the transcription of that material. Those narratives were processed in thematic grids, highlighting what was discussed in four thematic axes: Support, Articulation, Inter-federative, Stork Network (RC), and Arrangements and Agents of the PNAISC. The discourses of the mains agents of the research located in those four thematic axes constituted the Matrix of the Diverse Voices, which was presented and validated in an Evaluative Research Workshop, with the many agents involved in the process of formulation and implementation of the PNAISC. In that Workshop, a set of potentialities, fragilities, and propositions was formulated for each one of the thematic axes. Estudo da percepção dos coordenadores de saúde da criança dos estados e das capitais sobre os temas da Política Nacional de Atenção Integral à Saúde da Criança (PNAISC) / Study of the perception of child health coordinators of the states and capitals on the themes of the National Policy for Comprehensive Child Health Care (PNAISC)

Este artigo apresenta o estudo da percepção dos coordenadores de saúde da criança dos estados e das capitais a respeito dos temas da Política Nacional de Atenção Integral à Saúde da Criança (PNAISC) e dos fatores estruturais e organizacionais relacionados à sua Coordenação. O estudo foi realizado por meio de questionário estruturado, quantitativo, abordando a percepção subjetiva dos entrevistados (quantissubjetivo), aplicado aos coordenadores de saúde da criança dos estados e capitais. Eles avaliaram, em seus territórios, para cada tema pesquisado, os aspectos de importância, situação atual e expectativa de avanços. Este estudo proporcionou um importante panorama do desenvolvimento da saúde da criança nos territórios, na medida em que, a partir da análise dos dados, tornou possível identificar a percepção desses atores sobre os temas que se apresentam mais bem consolidados, bem como os que permanecem como desafios para a saúde da criança

This paper presents the study of the perception of child health coordinators of the states and capitals about the themes of the National Policy for Comprehensive Child Health Care (PNAISC) and the structural and organizational factors related to their Coordination. It was carried out through structured, quantitative questionnaire, addressing the subjective perception of the respondents, conducted with the child health coordinators of the states and capitals. They assessed, in their territories, for each studied theme, the aspects of importance, current situation, and advancement expectations. That study provided an important panorama of the development of the health of the child in the territories, insofar as, from the data analysis, it was then possible to identify the perception of those agents on the themes that seem to be the most well consolidated, as well as those which remain challenges for child health

Saúde mental e vulnerabilidade social: a direção do tratamento / Mental health and social vulnerability: direction of the treatment / Santé mentale et vulnérabilité sociale: la direction du traitement / Salud Mental y vulnerabilidad social: la dirección del tratamiento

O objetivo do texto é refletir sobre as relações entre saúde mental e vulnerabilidade social a partir do encontro da Psiquiatria Biológica com a Reforma Psiquiátrica. Problematizamos o conceito de saúde mental, o conceito de vulnerabilidade social, os manuais diagnósticos, as pesquisas epidemiológicas, tendo como pano de fundo as Políticas Públicas de Saúde. Identificamos a necessidade de superação da lógica técnico-científico tradicional incorporando a singularidade e saberes ligados aos sujeitos que são afetados pelo sofrimento.

The aim of this paper is to reflect on the relationship between mental health and social vulnerability at the intersection of biological psychiatry and the Brazilian psychiatric reform. The concepts of mental health and social vulnerability, as based on diagnostic manuals and epidemiological research, are taken up in the scope of public health policies. We identify the need to overcome the traditional technical and scientific logic to incorporate the singularity and knowledge related to human suffering.

L'objectif de cet article est de réfléchir sur les rapports entre la santé mentale et la vulnérabilité sociale à partir de la rencontre entre la Psychiatrie biologique et la Réforme psychiatrique. On problématise les concepts de santé mentale et de vulnérabilité sociale, les manuels de diagnostic, les recherches épidémiologiques, en prenant comme base les Politiques publiques de santé. On a identifié la nécessité de dépasser la logique technique et scientifique traditionnelle en intégrant la singularité et les savoirs liés aux sujets qui sont touchés par la souffrance.

El objetivo de este trabajo es reflexionar sobre la relación entre la salud mental y la vulnerabilidad social a partir del encuentro de la Psiquiatría Biológica y la Reforma Psiquiátrica. Problematizamos los conceptos de salud mental y de vulnerabilidad social, los manuales de diagnóstico, las investigaciones epidemiológicas, teniendo como telón de fondo a las Políticas de Salud Pública.
